In an effort to provide quality care for our patients in a timely manner, we’ve implemented a cancellation policy. This policy enables us to better utilize available appointments for our patients in need of dental care.
Cancellation of an Appointment
In order to be respectful of the dental needs of both Dr. Toubia and our patients, please be courteous and give our office 24-hours notice if you are unable to attend an appointment. Otherwise, you will be charged $65 for hygienist appointment & $150 per hour for Dr. Toubia’s appointment. We will make every effort to fill the cancelled appointment with someone else. If we are successful at short notice to do so, then cancellation fee will be waived. This is how we can best serve your needs.
No Show Policy
A failure to “not show” at the time of a scheduled appointment will be recorded in the patients’ chart as a “no show”. When there is a “no show” a cancellation fee of $150.00 per hour for treatment expected by Dr. Toubia will be imposed and there is a $65.00 cancellation fee imposed for hygiene appointments.
Payment is due at time of services. For large cases, a 50% down payment is required to reserve the time for treatment. For large cases, financial arrangements are available.
Fee for the services offered are the patients total responsibility. Insurance billing is done as a courtesy service only. Insurance verification of benefits and estimation of payment are not guaranteed. Actual payments by insurance companies may differ from our estimations. Denial of payment from insurance companies can happen for a variety of reasons, despite our verification of benefits. The patient has the final responsibility for the total fee of the services rendered.
